4th Annual JM Chubbuck Cancer Foundation Golf Tournament
The Joseph Michael Chubbuck Foundation (EIN: 46-3739937) invites you to Rome Country Club on Saturday-August 3, 2019 to golf for those fighting cancer. Interested golfers can register themselves or teams online at www.thejmcf.org under the Events tab or by calling 315-339-5993 weekdays 9am - 5pm.
Registration the day of the event begins at 7:30am. There will be shot gun start at 8:30am. The registration fee is $80 per player (teams of 4). Skins and Mulligans are optional. Last year 17 teams attended the event and the Foundation's goal is 20 teams for 2019.
Coffee, OJ, and donuts will be served at the start, followed by a light lunch at the turn, and a buffet (including desserts) at the conclusion. The top 3 teams will receive a Cash Prize and Awards will be presented for Longest Drive (male and female) and Closest to the Hole. Several nice door prizes will be available along with a Hole in One prize. 
All golfers will be given commemorative shot glass, golf tees, and team picture. REGISTRATION IS CURRENTLY OPEN at www.thejmcf.org under Events. Tournament proceeds will financially assist cancer patients living in or being treated within Oneida, Herkimer, Madison and Onondaga Counties. The foundation works with MVHS, SDMG, Bassett Cancer Center in Herkimer, Upstate Cancer Center and their local affiliate Offices, Dana Faber Cancer Center, CNY Hematology/Oncology, Roswell Park, MSK, and University of Rochester Wilmont Cancer Center. To date, the Foundation has assisted hundreds of patients and awarded approximately $180,000 thanks to support for the local community.
